Dear Medical Patient:
First of all, let me thank you for taking the time to come to this page. If you are reading this, most likely your doctor has referred you to Christine Rosche, MPH, CNS, Board Certified Nutrition Specialist & Biofeedback Therapist. To get started, please call our office to schedule your complimentary 30 minute introductory consultation (in person or by telephone). Please fill out the three Primary Intake Forms and USPS-mail/fax to the main office. Please read the first chapter of the Light Living Program Workbook (pdf) and fill out all questions on pages 1-20 to the end of the chapter and return the filled-in pages to our office. Please keep a daily food log using the sample form provided at the end of chapter one. Keep this form for a minimum of seven days before the first appointment.
